Q: What are your office hours?

Hours of Operation: 8 AM - 5 PM on Monday, Tuesday, Wednesday, Thursday

Office Hours: 8 AM - Noon on Friday

Procedures are performed on Monday and Wednesday afternoons and 8 AM - 5 PM on Fridays.

Office based procedures are performed during clinic hours.

Q: Do I need a referral to be seen at your clinic?

For patients seeking care from our office for interventional pain treatment, you will need a referral from your primary care physician. We will request medical records be provided along with diagnostic imaging such as x-rays, MRI, and CT also be submitted to our office prior to your appointment.

For aesthetic procedures or bio-identical hormone replacement therapy, call our office directly.

Q: Is there paperwork for me to complete prior to my appointment?

Yes. Patients should complete and return their intake paperwork two days prior to their appointment. A patient portal is available once your appointment is scheduled. You may also view information on our patient portal under the Patient Portal tab.

We accept most commerical insurance plans as well as Medicare.

Patient payments for co-pays, co-insurance, and regenerative and aesthetic procedures will be due at the time of service. We accept the following payment options:

While we are happy to offer many in office procedures, some procedures must be preformed in out patient surgical facilities.
